Position: Hospice Patient Care Manager
Job type: Permanent
Location: Oakland, CA
Job description:
  • The Hospice Patient Care Manager is responsible for the coordination of the clinical services of the assigned Hospice team(s).
  • Receives direction and supervision from the assigned Branch Office Director of Patient Care Services.
  • Provides direction and supervisor to Hospice RNs’ and other clinical and/or support positions as assigned.
  • Coordinates with the Hospice operations management team/to facilitate optimal/patient care and admissions.
Areas Of Responsibility:
  • Chairs the Interdisciplinary Group meetings.
  • Works closely with the Hospice Social Services and Bereavement Services to coordinate patient services.
  • Communicates regularly with the Medical Directors.
  • Collaborates with and coordinates scheduling, staffing and coverage of duties with other patient care coordinators, Branch Managers and with the direct Supervisor or assigned
  • Responsible to hire, orient, train, supervise, evaluate and provide direct supervision to assigned staff members.
  • Assists nurses in scheduling and in coordinating care with patient's physician and medical director by obtaining orders and information when needed.
  • Provides phone consultation to patients and their families regarding nursing care.
  • Demonstrates understanding and supports and implements Pathways Home Health and Hospice philosophy and principles and implements these in professional practice and aids assigned staff in understanding and implementing them.
  • Maintains current knowledge of regulations pertinent to the delivery of Hospice services and conducts team operations in a manner to be in compliance with such regulations.
Essential Physical And Mental Requirements:
  • Physical: Must be able to drive to off-site locations and patients' homes, move to other workstations do repetitive motions using equipment such as the telephone and computer.
  • Sensory: Must be able to hear, read, write and speak clearly in English to effectively communicate and relate with patients, staff, physicians and the community in person and via telephone.
  • Mental: Must be able to carry out responsibility for direction, control, and planning of a program, relate to others beyond giving and receiving instructions, have clinical knowledge and critical thinking ability to effectively oversee patient care consistent with standards and regulations.
  • Registered nurse, licensed to practice in the State of California
  • BS degree in nursing preferred.
  • Minimum of 2 years of field experience in Homecare.
  • Minimum of 1 year experience in the care & management of terminally ill patient's preferred.
  • Supervisory experience preferred.
  • Current California driver’s license and automobile insurance.
  • Current CPR Certificate.
