Title: Director of Patient Care Services
Location: Montrose, CA
Type: permanent
Job Description: 
•    The Director of Patient Care Services sliall have overall responsibility of coordination of
patient services.
•    Shall also be responsible for all activities relevant to the patient services furnished
including the development of personnel qualifications and the assignment of personnel.
•    The Director of Patient Care Services shall be available on the Agency's premises of
accessible by telecommunications during hours of operations.
•    Shall also devote a sufficient number of hours [at a minimum of forty (40) hours per
week) to assure the quality and adequacy of services provided and supervision of the
agency's staff.
Sunfimaty of essential job functions:
•    Working knowledge and understanding of the organizational make up of the agency.
•    Competency in general nursing theory especially in IV infusion and protocols,
medications, and practices, common knovi/ledge of diagnoses including signs and
•    Ability to direct and supervise the IDG team.
•    Knowledge and understanding of community resources available.
•    Provide advice and guidance to the agency's staff members;
â–     Regarding developing and maintaining appropriate policies and
standards of medical activities of the organization.
•    Provides information and education on:
"    Presents specific educational information at patient conference relevant
to case presentations.
•    Provides in-service education to staff.
•    Attends meetings and conferences:
â–     Participates in patient conferences as needed in order to review the care
being provided and to participate in patient care planning process.
•    Open case and visits patients in their homes as determined necessary and appropriate
by the Interdisciplinary team.
•    Working knowledge of federal, state and JCAHO guidelines/regulations.
â–     Serves as a liaison with the patient's primary physician and IDG as needed and
participate as an integral member of the team.
Minimum requirements    i
•    A RN with a Baccalaureate or higher degree in nursing.
•    Three (3) year health related experience within the last five (5) years in a home health,
hospice, and or primary care clinic or health facility,
•    At least one (1) year of supervisory or administrative capacity.
•    Current California RN license.
•    Current CPR certification.
•    Ability to resolve problems, set priorities, and ability to function independently.
•    Excellent written and verbal skills with demonstrated ability to deal tactfully with patients,
family members and staff.
•    Successful completion of our pre-employment screening:
Physical exam six (6) months prior to employment or within fourteen {14)
days of the date of employment and to be done every three (3) years
â–     TB PPD skin test six months (6) prior to employment of within fourteen
(14) days prior to employment and annually thereafter. If needed chest x-
day will be required if the employee is positive for TB PPD test.
