About the RN Role: Coordinate total nursing care for patients, participate in patient and family teaching, and provide leadership by working cooperatively with ancillary nursing and other patient team personnel in maintaining the highest standards for professional nursing practice in the clinical setting.

RN Responsibilities:

  • Conduct job responsibilities according to standards set forth in the SALMON Employee Handbook, Skilled Nursing Center policies, applicable federal and state laws, and professional standards.
  • Keep confidential all patient information received.
  • Maintain the standards of nursing care, implement the policies of the facility and nursing department, and follow all local, state, and federal rules and regulations.
  • Assess and document the patient’s condition and nursing needs. Set goals and prescribe appropriate nursing interventions to meet the patient’s physical, psychological, social rehabilitative and discharge planning.
  • Report pertinent observations regarding patients to the appropriate person (e.g., physicians, Nurse Manager, charge nurse and/or nursing supervisor, and appropriate family member). Record observations accurately and concisely.
  • Assign nursing care and other duties as needed to team members in accordance with the patient’s needs and team members’ capabilities and qualifications.
  • Supervise, direct, and evaluate LPNs and CNAs.
  • Initiate and lead team conferences to develop individualized nursing care plans.
  • Administer medications and perform treatments to assigned patients. Complete documentation of needs and treatment as required by local, state, and federal requirements.
  • Direct, supervise and evaluate nursing care provided to patients.
  • Perform within scope of practice as defined by state nursing practice act.
  • Assist with orientation of new personnel.
  • Assist team members in providing patient care, administer direct care when professional nursing skills are needed.
  • Perform other duties as assigned.

 

RN Requirements:

  • Graduate of RN from an accredited School of Nursing.
  • Current registration/license in Massachusetts.  Meets state CEU requirements.
  • CPR certified.
  • Ability to communicate in English effectively through verbal and written means.
  • Ability to work under stress and in emergency situations.

 

About the LPN Role:

Administer and coordinate nursing care for assigned patients, ensuring quality care, and supervision of personnel.

 

LPN Responsibilities:

  • Conduct job responsibilities in accordance with the standards set forth in the SALMON employee handbook, Skilled Nursing Center policies and procedures, applicable federal and state laws and applicable professional standards.
  • Keep confidential all patient information received.
  • Document patient’s condition; record in the clinical record and report all observed symptoms, reactions, treatments and changes in the patient’s condition.
  • Administer and chart medications as ordered by physician.
  • Assist physician with exams, treatment, special tests and procedures as defined in scope of practice.
  • Obtain and follow physician orders.
  • Direct care for assigned patients.
  • Orient new admissions and personnel to neighborhood.
  • Manage and plan daily and/or weekly nurse work assignments and breaks.
  • Ensure adequate staffing and direct duties of CNAs and nursing assistants
  • Communicate effectively with patients and their families, physicians and staff.
  • Participate in team conferences, development of nursing care plans, and team and shift reports.
  • Answer call lights, assist patients with personal hygiene and other daily living activities.
  • Assist in assessing, admitting, discharging and transferring patients.
  • Perform care within scope of practice as defined by state Nursing Practice Act.
  • Complete all documentation as required by local, state and federal guidelines.
  • Perform other duties as assigned.

 

LPN Requirements:

  • Graduate of LVN/LPN from an accredited School of Nursing.
